Author affiliation creation should not require adding city and state

Our submitters feel that the process of adding co-authors is an onerous process. We currently don't need or use the affiliation city and state information, so removing this requirement would make this step a bit less time consuming.

    This has been released but requires additional setup by your CIM.

    Or making the requirement of the any of the co-author fields configurable by society instead of determined by ScholarOne.
